Elizabeth L. Di, MD
Dr. Di is a board-certified family medicine physician who joined UF Health St. Johns after practicing family medicine at University Hospitals/Lake Health Mentor Family Practice in the greater Cleveland area. Dr. Di earned her bachelor’s degrees in Psychology and Latin American Studies from the University of San Francisco and completed her Master’s in Community and Social Psychology from the University of Massachusetts-Lowell. After receiving her medical degree from the Northeast Ohio Medical University in Rootstown, Ohio, Dr. Di completed her residency in Family Medicine at Case Western MetroHealth Medical Center in Cleveland, Ohio, where she was Education Co-Chief, centering pregnancy leader, and adjunct course instructor. Dr. Di has special interests in women’s health, long-acting reversible contraception, and the care of underserved patients. She is fluent in both English and Spanish.
Gëzim “Jimmy” Pajaziti, APRN
Gëzim “Jimmy” Pajaziti, APRN, is a certified nurse practitioner at UF Health Primary Care – Greenbriar. In addition to his years of clinical experience, he has worked as a respiratory therapist and patient care technician. His clinical interests include pulmonology, cardiology, obesity, and primary care. He strives to empower individuals in managing their health and well-being. He is a recent recipient of the DAISY Nursing Excellence Award, an international program that celebrates nurses for their exemplary patient care. He is excited to bring his passion for making a positive community impact through health education and overall improvement of health care outcomes in the St. Johns region.
Mark S. Michaels, MD
Mark Michaels, MD, is vice president and chief medical officer for UF Health St. Johns’ team of employed ambulatory providers. As chief medical officer, Dr. Michaels serves as the clinical leader for all primary care ambulatory service lines. He works with all staff to ensure the highest standards of patient care are provided from every UF Health St. Johns ambulatory venue. Maintaining the highest standards for patient safety, medical care and satisfaction are his top priorities. Dr. Michaels represents all the providers and also sees patients at UF Health Primary Care – Greenbriar. Dr. Michaels is an American Board of Family Medicine certified physician. He received his medical degree from St. George’s University, a master’s degree in basic medical sciences from Wayne State University, and a bachelor’s degree in brain, behavior and cognitive science from the University of Michigan.
Daniel Landis, MD

Dr. Daniel Landis is a dedicated internal medicine physician who sees patients 18 and older. He is committed to making every person in his care feel heard and understood, believing that communication is the key to making the best possible health care decisions. As a resident, Dr. Landis committed himself to providing high-quality care. He was selected by New York Magazine as one of their Top Doctors in 2025. He has treated patients from diverse backgrounds with compassion and a collaborative spirit. Dr. Landis earned his medical degree from the American University of the Caribbean School of Medicine in Sint Maarten, and completed his internal medicine residency at Nassau University Medical Center in East Meadow, New York.
