Who can benefit from home care?
Home care is not just for seniors or older adults. Home health services are beneficial to those who need continued medical treatment at home after recovering from surgery, ongoing medical care for long term illnesses, chronic disease management, medication education, or assistance with daily tasks.
What should I expect as a new patient?
You will receive a welcome call from our intake department to verify the information we’ve been provided by your provider. A clinician will follow up to schedule an initial visit that is convenient to you.
What should I expect at my first appointment?
Most often, a nurse or other skilled health care professional will schedule an appointment to visit the home and complete an evaluation. After the evaluation, and under the direction of a physician, a specialized care plan will be created that is specific to the patient's needs and goals. A family member or loved one is welcome to participate in these appointments, if the patient prefers.
